Common Hormonal Causes of Fatigue in Women

Hormone changes are among the most overlooked contributors to persistent fatigue. These include:

  • Declining estrogen and progesterone during perimenopause, which disrupts sleep architecture and energy regulation
  • Low rogesterone, which can cause light, non-restorative sleep even when hours are adequate
  • Thyroid dysfunction, including Hashimoto’s thyroiditis — the most common thyroid condition in women — which can cause profound fatigue even when TSH is in the “normal” range
  • Adrenal and cortisol dysregulation from chronic stress
  • Low testosterone, which affects energy, motivation, and stamina in women as well as men
  • Insulin resistance and blood sugar instability, which cause energy crashes and afternoon slumps
  • Iron deficiency, particularly in women with heavy periods

How We Evaluate and Treat Fatigue

There is no single lab or single answer for fatigue. Our approach starts with listening — to your full symptom picture, your history, your sleep, your stress patterns, and your goals. From there, evaluation may include:

  • Comprehensive thyroid panel (TSH, free T3, free T4, thyroid antibodies)
  • Hormone evaluation (estrogen, progesterone, testosterone, DHEA)
  • Metabolic labs including fasting insulin, blood glucose, and HbA1c
  • Iron, ferritin, B12, vitamin D, and other nutritional markers
  • Adrenal and cortisol assessment when clinically appropriate

Questions, Answered With Care

We understand that persistent fatigue can feel frustrating, confusing, and easy to dismiss. Here are answers to common questions about fatigue, hormones, thyroid health, and personalized care.

Yes. Estrogen, progesterone, thyroid hormones, testosterone, and cortisol all play significant roles in energy regulation, sleep quality, and mitochondrial function. When these are out of balance, fatigue is a natural consequence — not a character flaw or a sign of laziness.

Standard TSH testing captures many thyroid issues but not all. Some patients feel consistently better when free T3 and free T4 are optimized within range, even when TSH alone looks acceptable. Thyroid antibodies can also indicate Hashimoto’s, which affects energy and overall wellbeing. We look at the full picture.

Some energy changes come with age, but persistent, disabling fatigue is not something you simply have to accept. In our experience, most women with significant fatigue have identifiable contributors that can be addressed — and many feel substantially better with targeted care.
