Why Athletes Choose a Specialized Surgeon
General plastic surgeons treat athletes the same way they treat any patient. We don’t. Lean, muscular bodies require different surgical planning:
- Lower body fat changes everything. Liposuction technique, incision placement, and scar planning all shift when there's no fat layer to hide behind.
- Muscle density affects implant choice. A developed pec changes how breast implants sit. A thick neck changes how a chin implant reads on camera.

Gynecomastia Surgery for Bodybuilders
Gynecomastia is the most common procedure we perform on male athletes. Whether it’s pubertal tissue that never went away, hormonal fluctuation from cycling, or post-cycle rebound, glandular tissue doesn’t respond to training or diet. It has to come out.
- Direct gland excision combined with high-definition liposuction of the chest and flanks for a sculpted, athletic result
- Hidden periareolar incisions that disappear once healed
- Revision gynecomastia for botched surgeries done elsewhere — including crater deformity, residual tissue, and asymmetry corrections

Most patients are back to lower-body training in a week and chest work in four to six weeks.
High-Definition Liposuction and Ab Etching
Standard liposuction removes fat. High-definition liposuction (also called VASER lipo, 4D lipo, or ab etching) sculpts the underlying muscle anatomy by selectively removing fat over the linea alba, linea semilunaris, and serratus to reveal the definition underneath.

Done well, it looks like genetics. Done poorly, it looks obvious. Choose accordingly.
Mini Tummy Tuck for Athletes with Mild Loose Skin
Most patients who think they need a tummy tuck actually need liposuction, and most who need a full tummy tuck aren’t athletes. But there’s a narrow group of athletes who are genuinely good candidates for a mini tummy tuck — and almost no one in the industry explains who they are. You may be a candidate if:
- You've lost significant weight or finished a major cut and have mild loose skin below the navel that won't tighten further
- You have a small amount of lower abdominal skin laxity from prior pregnancy combined with a strong, trained core
- You have mild lower-abdominal diastasis or a small lower-belly pooch that persists despite a defined upper abdomen
- Your skin laxity is confined to the area below the belly button — not above
You’re probably not a candidate if:
- The "loose skin" is actually visceral fat (a firm, round lower belly that doesn't pinch)
- You have laxity above the belly button — that requires a full tummy tuck
- You haven't yet reached your stable training weight and body composition
The mini tummy tuck removes a small ellipse of skin through a short, low incision (shorter than a C-section scar in most cases), tightens the lower abdominal wall, and leaves the belly button untouched. Recovery is faster than a full tummy tuck — most athletes return to lower-body work within two weeks and full core training within six.
Combined with hi-def liposuction of the upper abdomen and flanks, the result is a fully sculpted midsection that no amount of training alone can produce in patients with this specific anatomy.
Motiva Breast Augmentation for Female Athletes
Female physique competitors, CrossFit athletes, and lifters face a specific problem: low body fat means less breast tissue, and standard implants placed under a developed pec can distort during contraction and look unnatural at low body fat. We use Motiva Ergonomix implants because they:
- Move naturally with the body during athletic activity
- Have a softer profile that reads natural on lean frames
- Offer the lowest reported rates of capsular contracture and ALCL
- Come in sizes appropriate for athletes who don't want a cartoonish look
Placement (subglandular vs. submuscular vs. dual-plane) is decided based on your training, pec development, and aesthetic goals — not a one-size-fits-all default.
Lip Lift Surgery — End the Filler Cycle
If you’ve been getting lip filler every six months for years, you already know the math: it adds up to thousands of dollars annually, the results plateau, and over-filled lips on a lean, defined face look obvious. A bullhorn lip lift is a permanent surgical solution that:
- Shortens the philtrum (the space between nose and upper lip)
- Increases visible vermilion (pink lip show) without volume
- Creates a more youthful, defined lip shape that holds up at low body fat
- Eliminates the ongoing cost and maintenance of filler
The scar sits hidden at the base of the nose and fades to near-invisible in most patients. One procedure, done once.
Chin and Jaw Implants for Facial Balance
You’ve built a strong frame. Does your face match it? A lot of athletes spend years developing their physique only to feel like their jawline doesn’t keep up — especially on camera, on stage, or in photos. Custom or off-the-shelf chin and jaw implants restore proportion:
- Chin implants for projection and definition, especially helpful for patients with a weak or receding chin
- Jaw angle implants for width and angularity that reads as masculine and powerful
- Combined chin + jaw for a fully reshaped lower face
- Genioplasty (sliding chin advancement) as an alternative to implants when the bony structure needs to move
Modern implants are made of medical-grade silicone or porous polyethylene, placed through hidden intraoral incisions, and sized to match your existing facial structure rather than overpowering it.
Rhinoplasty for Athletes
The nose is the centerpiece of the face. For athletes who’ve spent years sculpting everything below the neck, a nose that doesn’t fit the face can undercut the whole look — especially in stage lighting, on camera, or in physique photography where the face is part of the package. Our rhinoplasty approach for athletes focuses on:
- Refinement, not transformation. Subtle changes that improve proportion without looking "done"
- Functional preservation. Athletes can't afford compromised breathing — septoplasty and internal valve work are integrated when needed
- Masculine vs. feminine planning. Preserving (or creating) a strong dorsum and defined tip in male patients; softer refinement in female patients
- Revision rhinoplasty for patients unhappy with prior results
- Ethnic rhinoplasty that refines without erasing identity
Recovery for rhinoplasty is mostly facial swelling — lower-body training resumes quickly, but heavy lifting and anything that elevates blood pressure to the face is restricted for several weeks.
Facelift and Facial Rejuvenation
Athletes age differently. Low body fat, sun exposure from outdoor training, and the volume loss that comes with sub-10% body fat can age the face faster than the body. Many lifelong athletes look ten years younger from the neck down and feel like their face is catching up too quickly. Our facelift approach is built for active patients:
- Deep plane facelift for natural, long-lasting results that don't have the "pulled" look
- Neck lift to restore the jawline definition lean training should reveal
- Combined with fat grafting (see below) to restore volume rather than just tightening skin
- Shorter-recovery options like mini-lifts or energy-based skin tightening for younger patients not ready for full surgery
The goal is looking like a well-rested, well-trained version of yourself — not someone who had work done.
Fat Grafting with Allograft — Natural Volume That Lasts
This is one of the most versatile tools in modern plastic surgery, and it’s especially valuable for athletes. Fat grafting takes fat from areas you don’t want it (flanks, lower back, inner thighs) and transfers it where you do want volume. Combined with allograft tissue (donor-derived collagen matrix), the results are more durable and natural than fat or implants alone. Where fat grafting matters for athletes:
- Making implants look natural. A breast implant or pec implant on a lean frame can look obvious at the edges. A layer of fat grafted over the implant softens the transition, hides the implant edge, and creates a result that reads as tissue, not hardware. Same principle applies to chin, jaw, and calf implants.
- Facial volume restoration. Lean athletes lose facial fat first. Grafting to the temples, cheeks, and tear troughs restores the volume that low body fat strips away — a more youthful, healthy face without filler maintenance.
- Pec and deltoid enhancement. For male athletes who want more upper-body fullness without implants, fat grafting can add subtle volume to the upper pec and deltoid cap.
- Hand and décolletage rejuvenation. Often overlooked, these are the giveaway areas for athletes who otherwise look young.
- Scar and contour correction. Grafting can fix divots from old surgeries, including crater deformity from prior gyno procedures.
Why we use allograft alongside fat:
Pure fat grafting has variable take rates — typically 50 to 70 percent of the transferred fat survives long-term. Adding allograft tissue (acellular dermal matrix products) provides a scaffold that improves graft survival, adds immediate volume, and produces more predictable results, especially in lean patients with limited donor fat to work with.

Will you work with me if I'm on or have used performance-enhancing compounds?
We treat every patient with discretion and without judgment. We do need an accurate medical history for surgical safety — blood pressure, hematocrit, and cardiac risk all matter for anesthesia. What you share is confidential and used only to plan a safe procedure.
How soon can I get back to training?
Depends on the procedure. Lower body work resumes within days for most upper-body procedures. Chest training after gyno surgery is typically four to six weeks. Full intensity returns at six to eight weeks for most procedures.
Do you have experience with competitive physique athletes?
Yes. We’ve worked with bodybuilders, physique and bikini competitors, and athletes from across the Pacific Northwest.
I'm not sure if I'm in perimenopause. Can Jade help me figure that out?
Yes. Identifying and evaluating perimenopause is one of the most common reasons patients come to Alpenaura. Perimenopause is a clinical diagnosis — labs alone do not confirm or rule it out — and the conversation about symptoms, timing, and history is often more informative than any single lab result.
I had gyno surgery somewhere else and it looks bad. Can you fix it?
Revision gynecomastia is one of our most common procedures. Crater deformity, residual tissue, and asymmetry are all correctable in most cases.
