The gonial angle is the point where the lower jaw turns upward toward the ear — and a wide, square gonial angle is one of the most recognizable masculine features. Gonial angle resection reduces this angle from the inside of the mouth (no external scars), narrowing and softening the lower face.Often combined with masseter muscle reduction and jawline tapering, this procedure transforms a square jaw into a softer, more oval lower face.When combined with genioplasty, gonial angle resection completes the feminization of the lower third of the face — usually one of the most impactful changes patients notice in their results.

We understand that choosing a procedure is a deeply personal decision. Here are answers to some of the most common questions about treatment, recovery, results, and the overall Alpenaura experience.
You absolutely choose. FFS is highly individualized — some patients want comprehensive feminization in one or two surgical sessions; others focus on the one or two features that affect them most. At consultation, we identify which procedures will have the greatest impact for your face and build a plan around your priorities and budget.
Both approaches are safe. Combining procedures means one recovery period and lower total cost, but a longer surgery and bigger initial recovery. Staging means shorter individual recoveries and the ability to see results before committing to the next step. We'll discuss which approach fits your situation.
Initial recovery — when you're comfortable in public and most major swelling has resolved — is typically 3–4 weeks. Continued swelling resolution and tissue settling happens over 3–6 months, with final results visible at 12 months and beyond. Bone procedures (brow, chin, jaw) take longer to fully settle than soft tissue work.
Most FFS procedures are designed to avoid visible scars. Brow contouring is done through an incision hidden at or behind the hairline. Genioplasty and gonial angle resection are done from inside the mouth — no external scars. Rhinoplasty uses either a tiny incision on the columella (open) or fully internal incisions (closed). We discuss scar placement in detail for each procedure at consultation.
Non-surgical options can soften features temporarily and are a reasonable starting point for some patients, but fillers can't reduce bone, and they require ongoing maintenance and cost. FFS makes lasting structural changes — particularly to bone — that injectables cannot achieve. Many patients see both surgical and non-surgical approaches as part of their overall plan.
Insurance coverage for FFS has improved significantly in recent years, particularly in Oregon, Washington, and California. Many plans now cover FFS when documented as medically necessary, though coverage is more variable than top surgery. Our team helps with prior authorization and appeals when needed.
For comprehensive FFS, most out-of-town patients plan 10–14 days in Bend post-op. This allows for swelling to begin resolving, removal of any sutures or splints, and the first post-op visit before traveling home. Patients having a single procedure can often travel home sooner.
Most patients return to remote or non-public-facing work at 2–3 weeks. Public-facing work typically resumes at 4–6 weeks, when bruising has resolved and swelling is significantly improved. Final result reveal is gradual rather than dramatic.
