GENDER-AFFIRMING FACIAL SURGERY

Facial Feminization Surgery in Bend, Oregon | FFS for the Pacific Northwest

The face carries an enormous amount of gendered information — often more than any other part of the body. For many transfeminine and nonbinary patients, facial feminization surgery (FFS) is the most impactful step in being seen, every day, as themselves. Our Bend, Oregon practice offers comprehensive FFS for patients across the Pacific Northwest, combining the procedures that work in concert to soften and feminize facial features.

PERSONALIZED APPROACH

Facial Feminization Surgery in Bend, Oregon | FFS for the Pacific Northwest

We believe FFS should never be a one-size-fits-all package. Every face is different, every patient’s goals are different, and the best surgical plan is the one built around you. During your consultation, we review your anatomy in detail, talk through what feels most important to address, and design a plan that may include any combination of the procedures below.

Rhinoplasty (Feminizing Nose Surgery)

Rhinoplasty (Feminizing Nose Surgery)

A feminizing rhinoplasty refines the shape, size, and proportions of the nose to harmonize with softer, more feminine facial features.

  • Reducing a dorsal hump along the bridge
  • Refining the nasal tip for a softer, slightly upturned appearance
  • Narrowing a wide bridge or wide nostrils
  • Reshaping the nose to fit better with planned brow and chin changesBecause the nose sits at the center of the face, rhinoplasty results depend heavily on what’s happening around it. We plan rhinoplasty in the context of your full FFS plan rather than in isolation — the right nose for your face depends on what your brow, cheeks, and chin are doing too.
Brow Contouring (Forehead Feminization)

Brow Contouring (Forehead Feminization)

The brow ridge — the bony prominence above the eyes — is one of the strongest gender markers in the face. Brow contouring reshapes this area to create a smoother, more feminine forehead.

  • Type 1 (burring): For patients with minimal bossing, the bone is carefully reduced from the surface.
  • Type 3 (forehead reconstruction): For patients with significant frontal bossing and a hollow frontal sinus, the front wall of the sinus is removed, reshaped, and replaced — allowing significant setback while preserving sinus function.Brow contouring is often combined with a hairline advancement and brow lift to create a fully softened upper third of the face. We’ll discuss which approach fits your anatomy and goals at consultation.
Genioplasty (Chin Feminization)

Genioplasty (Chin Feminization)

A more feminine chin is typically narrower, shorter, and more rounded than a masculine chin. Genioplasty reshapes the chin bone itself — not just the soft tissue overlying it — to create lasting feminization.

  • Narrow a wide chin
  • Reduce vertical chin height
  • Round off a square or pointed chin
  • Reposition the chin slightly to improve facial proportionsBecause the chin works in harmony with the jawline, genioplasty is often combined with gonial angle resection for a fully feminized lower face.
Orbital Rim Reshaping

Orbital Rim Reshaping

The bony rim around the eyes plays a subtle but important role in how the face reads. Masculine orbital rims tend to be more prominent and angular, while feminine rims sit higher, smoother, and more open. Orbital rim reshaping reduces and softens this bony border — usually performed alongside brow contouring through the same surgical access — to open up the eye area and create a softer, more feminine eye frame.

Gonial Angle Resection (Jaw Feminization)

Gonial Angle Resection (Jaw Feminization)

The gonial angle is the point where the lower jaw turns upward toward the ear — and a wide, square gonial angle is one of the most recognizable masculine features. Gonial angle resection reduces this angle from the inside of the mouth (no external scars), narrowing and softening the lower face.Often combined with masseter muscle reduction and jawline tapering, this procedure transforms a square jaw into a softer, more oval lower face. When combined with genioplasty, gonial angle resection completes the feminization of the lower third of the face — usually one of the most impactful changes patients notice in their results.

Planning Your FFS in Bend, Oregon

Planning Your FFS in Bend, Oregon

Facial feminization is one of the most personal surgical journeys a patient can take, and we treat it that way. Your consultation includes:

  • A detailed facial analysis with attention to your specific goals
  • Discussion of which procedures will have the greatest impact for your face
  • Honest conversation about what can and cannot be changed surgically
  • A realistic timeline, including whether to stage procedures or combine them
  • Clear discussion of recovery, scarring, and expected resultsWe see FFS patients from across the Pacific Northwest — Portland, Eugene, Seattle, Boise, Spokane — and from across the country. Bend’s calm setting, clean air, and beautiful surroundings make it a particularly restorative place to recover from facial surgery, and we help coordinate lodging and travel logistics for out-of-area patients.

  • A return to social activity around three to four weeks

  • Final results settling at twelve months and beyondWe see you for follow-ups at regular intervals throughout this period and remain available between visits for any questions or concerns.

Questions,
Answered With Care

We understand that choosing a procedure is a deeply personal decision. Here are answers to some of the most common questions about treatment, recovery, results, and the overall Alpenaura experience.

You absolutely choose. FFS is highly individualized — some patients want comprehensive feminization in one or two surgical sessions; others focus on the one or two features that affect them most. At consultation, we identify which procedures will have the greatest impact for your face and build a plan around your priorities and budget.

Both approaches are safe. Combining procedures means one recovery period and lower total cost, but a longer surgery and bigger initial recovery. Staging means shorter individual recoveries and the ability to see results before committing to the next step. We'll discuss which approach fits your situation.

Initial recovery — when you're comfortable in public and most major swelling has resolved — is typically 3–4 weeks. Continued swelling resolution and tissue settling happens over 3–6 months, with final results visible at 12 months and beyond. Bone procedures (brow, chin, jaw) take longer to fully settle than soft tissue work.

Most FFS procedures are designed to avoid visible scars. Brow contouring is done through an incision hidden at or behind the hairline. Genioplasty and gonial angle resection are done from inside the mouth — no external scars. Rhinoplasty uses either a tiny incision on the columella (open) or fully internal incisions (closed). We discuss scar placement in detail for each procedure at consultation.

Non-surgical options can soften features temporarily and are a reasonable starting point for some patients, but fillers can't reduce bone, and they require ongoing maintenance and cost. FFS makes lasting structural changes — particularly to bone — that injectables cannot achieve. Many patients see both surgical and non-surgical approaches as part of their overall plan.

Insurance coverage for FFS has improved significantly in recent years, particularly in Oregon, Washington, and California. Many plans now cover FFS when documented as medically necessary, though coverage is more variable than top surgery. Our team helps with prior authorization and appeals when needed.

For comprehensive FFS, most out-of-town patients plan 10–14 days in Bend post-op. This allows for swelling to begin resolving, removal of any sutures or splints, and the first post-op visit before traveling home. Patients having a single procedure can often travel home sooner.

Most patients return to remote or non-public-facing work at 2–3 weeks. Public-facing work typically resumes at 4–6 weeks, when bruising has resolved and swelling is significantly improved. Final result reveal is gradual rather than dramatic.

Bring photos that reflect features you find feminine, photos of yourself from different angles, and a list of which features bother you most. There's no need to come in knowing exactly which procedures you want — our job is to help you understand what's possible. We also welcome questions about staging, budget, recovery, and timing.
